To be completed and signed by the model's parent.

I grant permission to Lisa Johnson Photography the irrevocable and unrestricted right to take and reproduce the photographs and/or video images taken of me and members of my family, for the purpose of publication, promotion, illustration, advertising, sale, or trade, in any manner or in any medium. I hereby release Lisa Johnson Photography and its legal representatives for all claims and liability relating to said images. I waive my right to any compensation.

Lisa Johnson Photography retains copyright ownership of all images.

Print Release. Lisa Johnson Photography grants permission to model to make an unlimited number of prints of the images provided for personal (non-commercial) use. The images provided are licensed for personal purposes only, including but not limited to: printing, copying, emailing & web publishing. This license does not include use that results in financial gain, including but not limited to: advertising, competitions, stock photography, commercial endeavours, print sale profits, or resale of any nature.  You may not under any circumstances alter, edit or change images.
